Bilingual Nurse and Health Coach

About the role

The Bilingual Nurse and Health Coach will work remotely full‑time to deliver patient education, health coaching, and care coordination in English and a second language (e.g., Spanish). The role blends clinical nursing expertise with motivational support to help patients manage chronic conditions, adopt preventive practices, and achieve lifestyle goals.

Key responsibilities

• Conduct telehealth or phone consultations, review patient histories, and develop individualized care plans.
• Provide health coaching, motivational interviewing, and lifestyle counseling in both languages.
• Collaborate with providers, virtual medical assistants, and multidisciplinary teams to ensure seamless care delivery.
• Document all interactions accurately in electronic health records and adhere to HIPAA and privacy standards.
• Monitor patient progress, offer ongoing encouragement, and assist patients in navigating healthcare resources.

Required profile

• Active, unrestricted Registered Nurse (RN) license with strong clinical assessment and patient‑education abilities.
• Proven experience in chronic disease management, preventive care, and health coaching.
• Excellent written and verbal communication skills in English and a second language.
• Ability to work independently, manage time effectively, and collaborate remotely with clinical teams.

Required skills
