Lifestyle Coordinator – Aged Care

About the role

The Lifestyle Coordinator’s primary goal is to develop and oversee a Lifestyle program that reflects client preferences and enables individuals to maximise their abilities while respecting each person’s physical, social, cultural and religious background. This role mentors and leads the delivery of programs that become focal points of social activity and emotional connection for residents.

Key responsibilities

• Establish positive, supportive communication with each client and their next of kin to produce activity assessments and care plans reflective of resident interests and abilities.
• Develop, implement and evaluate individual, social and leisure care plans, mentoring the team on best delivery practices.
• Identify a variety of entertainment options within an agreed budget and incorporate them into a monthly program, promoting resident engagement.
• Build positive relationships with support staff, nurses and volunteers to ensure a comprehensive, compassionate, holistic approach to social, emotional and cultural wellbeing.
• Ensure lifestyle coordination complies with organisational policies, managing risks and hazards promptly.

Required profile

• Empathy and ability to relate to elderly clients while working effectively with health professionals and service providers.
• Demonstrated success in an aged care setting, including experience in assessment and equipment prescription.
• Experience in the development and delivery of therapy‑based programs.
• Commitment to being a productive, positive team member with excellent interpersonal, communication and time‑management skills.
• Knowledge of community‑based programs and a flexible approach to working within them.
