Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

This remote position supports a busy US‑based cardiology team by guiding patients through their care journey. The role focuses on phone‑based communication, scheduling, documentation, and coordination of clinical workflows for a high‑volume caseload.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR platforms.
• Conduct telephonic outreach to reinforce care instructions, answer questions, and coordinate next steps.
• Track testing schedules, remote monitoring data, and follow‑up appointments for 150+ patients.
• Escalate clinical concerns to the appropriate provider while remaining within scope (no diagnosing or prescribing).
• Maintain accurate, HIPAA‑compliant documentation and support billing and insurance coordination.

Required profile

• Clinical credential such as CNA, MA, RN, or equivalent.
• Minimum 3 years experience in healthcare, telehealth, or medical office settings.
• Proven ability to handle high‑volume, remote care coordination with strong phone etiquette.
• Self‑directed, organized under pressure, and comfortable adapting to evolving processes.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
• Knowledge of HIPAA compliance standards.
• Basic understanding of billing documentation and insurance coordination.
