Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

This is a full‑time remote position supporting a busy US‑based cardiology team. The Cardiology Patient Navigator will handle patient communication, scheduling, care coordination, and clinical documentation for a high‑volume caseload.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, follow‑up appointments, and remote patient monitoring workflows.
• Coordinate with the clinical team to ensure continuity of care for 150+ patients.
• Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA compliance.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical credential (CNA, MA, RN) with 3+ years in healthcare, telehealth, or medical office settings.
• Strong phone communication skills and a patient‑first mindset.
• Ability to manage a high‑volume, remote workload with warmth and precision.
• Self‑directed, proactive communicator comfortable with evolving processes.
